A new survey of 21 Large Academic Medical Centers in the USA has revealed some sobering facts. We assume that QC processes have evolved from the antique practices of the 1950s and 1960s, particularly since the introduction of "Westgard Rules" in 1981 and other evidence-based optimization techniques in subsequent decades.
